You may qualify for our Homelessness Response Team (HRT).

What is HRT?

VOA Northern New England’s Homelessness Response Team (HRT) is part of the MaineCare’s HOME Provider Program for those who are experiencing long-term homelessness and chronic health issues. We work with individuals to access the necessary resources to transition out of homelessness and into stable, permanent housing.

The program offers client advocacy, case management, benefit establishment, linkage to health, mental health, and substance use services, linkage to stable housing and all other supportive services as needed per the individual served.

What we offer:

· Housing navigation and support

· Care coordination

· Health promotion

· Comprehensive Transitional Care

· Individual and Family Support

· Peer support

· Referral to community and support services

· Helping to connect to MaineCare and determining MaineCare eligibility

· Proactive Identification of Members who Need HOME Services through the community contacts and outreach

· 24/7 on-call access to HRT Team for emergency situations

Our goal is to support you to obtain housing and KEEP your housing by offering comprehensive case management, housing and peer support to help you reach your housing goals utilizing your strengths to overcome housing and health barriers. Together we will work on these goals by addressing areas of your life that may be a challenge for you.

HRT Team:

The HRT staff is committed to honoring member choice, fair housing, housing first, nondiscrimination, low-barrier access, evidence-based practices, trauma-informed care, and skill development. The HRT staff includes a Clinical Manager, Case Manager, and Peer Support/Housing Navigator. Our team is listed below with some of their responsibilities.

· Clinical Manager – Kristen Jiorle LADC, CCS: Program operation and management, coordinated entry, clinical assessment/intake, periodic review.

· Case Manager: Connections to all supportive services needed to lead a healthier life. Coordinates referrals and supports members in accessing and attending appointments. Provides continued support to the member once housed.

· Peer Support/Housing Navigator: Intentional and Recovery Peer support, RentSmart training, housing search, applications, lease-up, move-in, and continued support to members once housed.

Where can I get HRT Services?

The HRT team currently provides support/services to adults in Kennebec and Somerset Counties.

How long are HRT Services?

Adults are enrolled in HRT once they consent and are considered eligible to be in the program. Once enrolled, our HRT supportive services are available as long as the members continue to meet the eligibility criteria based on a standardized assessment.
