Creating Opportunities
The West Side Place Behavioral Health Program consists of three residents who have mental health diagnoses with symptoms that significantly impact their lives, resulting in the need for long term support. The resident’s measurements of success in the program will range from an ability to engage in goal areas that support them to increase their levels of independence in a community residential setting, and/or to a decrease in inpatient psychiatric hospital admissions or emergency department visits due to psychiatric impairment.
This program supports the individuals with activities of daily living, based on their needs and as outlined in their individual support plans. Daily living skills include items such as self-care in hygiene, relationship building/peer interactions, medication oversight, and community integration, along with numerous other items. Support required will range from role modeling, verbal or physical prompting, to total care.
