The Community Corrections Transition & Reunification Program offers intensive and integrated pre and post release case management services to offenders currently incarcerated at the Maine Correctional Center.  This program has successfully established collaborative relationships with a multitude of service providers throughout the state of Maine to address the biological, emotional, sociocultural, socioeconomic and spiritual needs of offenders’ transitioning into society.

Coordination of Transitional Case Plans has been advanced through utilization of needs and risk assessment tools, the practice of Community Building with public and private service providers, and the incentive of intensive case management advocacy in an effort to provide a synthesized re-entry in to the community.  Emphasis has been on developing statewide relationships with Probation Officers, employers, mentors, as well as, treatment and resource providers.

Each offender has needs upon transition unique to their circumstances and the county they will be transitioning to.  The Transition and Reunification Program has compiled a library of strategies and resources to address challenging geographical locations encountered when working with this diverse population.  This program has adopted a holistic approach to reintegration through the practice of five governing principles of Empowerment, Meaningful & Responsible Choices, Respect & Dignity, Supportive Environment, and Shared Responsibility.
